HPV

Diagnosis of HPV will be excluded from coverage.

Right bundle branch block

Depends on the following:

The degree of the blockage: Incomplete or Complete? Incomplete usually standard class (no surcharge) assuming no other cardiovascular abnormality. The underwriter will review doctors' records and may require an ECG.

If complete: Congenital or Acquired? Best case ok/std, worse case rated with shortened benefit period ( 75% surcharge and 2 yr Benefit Period limitation).

How does passive income affect benefits?

Guardian will reduce the applied for monthly benefit amount using the following formula:

Determine the annual earned income and multiply 10%; this is the amount of passive income allowable without reducing the benefit amount. Passive income exceeding 10% of earned income offsets the available monthly benefit benefit by 50%.

Is Acutane excluded?

No. Presuming that the Acutane prescription is for ance treatmentm, no adverse underwiting will result.

Is LASIK surgery considered "Invasive"?

LASIK procedures are considered "non-invasive". Guardian offers two rate classes for medical doctors; surgical or "invasive" physicians pay a higher premium.

Too heavy for his height?

5'4" and 245 pounds equals an unisurable weight.

Is this Diabetic insurable?

Is this 44 year old male insulin dependent diabetic insurable? He is six feet tall and weighs 160 pounds; he was diagnosed six years ago; he has been prescribed insulin three times a day; one unit per 15 grams of carbs and seven units before bedtime. Otherwise he is in good health.

Guardian underwriter's response:

"44 male with Type 1 DM..
50%, 5 year BP no other benefits... If there has been good control and no residual effects from the DM"

Occupational class for Mohel?

Deductions and Section 179 Expense
